I prefer to write my own goals. I use assessment data that I've collected on the student. This could be results from district testing such as MAP scores, iReady or standardized testing such as the KTEA or WJ IV. 

Depending on the student, It may give or create something more informal. Some students don't do well with standardized testing due to anxiety, not being familiar with the surroundings or comfortable with the test examiner. I will sometimes gamify an informal assessment using online web applications like Prodigy, or create a quiz in Kahoot!. In Khan Academy and CK12, you can select specific question and assign a quiz. It really depends on the learner. 

I have an app that I use which breaks down the state standards. From there I will use the data in combination with the standard that applies to modify I plan to meet it. For most student, I'll describe a specific task or goal that demonstrates how they have met that particular part of the standard. Then break that task down into objectives.  Best practice is to create SMART Goals (Specific, Measurable, Attainable, Time-bound).
